We are committed to providing you with the best possible care. If you have dental or medical insurance, we are eager to help you receive your maximum allowable benefits. In order to achieve these goals, we need your assistance, and your understanding of our payment policy.
Payment is due on the date of service. We accept cash, checks, MasterCard, Visa or Discover. We also participate in the Care Credit program and can assist you with completing an application or go to carecredit.com for more information.
We accept most insurances and are participating providers with many companies. If you have insurance, an estimate of your liability will be given to you and payment of that part is due at the time of service. This, however, is only an estimate and not a guarantee of payment by your insurance company. Any balance remaining after insurance has paid will be billed; credit balances will be refunded. (Valley Oral & Facial Surgery has the discretion to charge a processing fee on all refunds.)
We will gladly discuss your proposed treatment and answer any questions relating to your insurance.
You must realize that:
- Your insurance is a contract between you, your employer and the insurance company. We are not a party to that contract.
- Not all services are a covered benefit in all contracts. Some insurance companies arbitrarily select certain services they will not cover.
We must emphasize that as a care provider, our relationship is with you, not your insurance company. While the filing of insurance claims is a courtesy that we extend to our patients, all charges are your responsibility from the date the services are rendered.
Statements are generated monthly regardless of your insurance status. Most insurance companies will respond within four to six weeks. Please contact your insurance company if your claim is pending beyond 30 days. Any remaining balance after your insurance has paid is due in-full within 30 days.
